You may have out-of-network benefits through your insurer or HSA/FSA. Check with your insurance plan to see if this applies to you. If you have these benefits, you will first self-pay and then request an itemized receipt (or super bill) and submit this to your insurer for reimbursement. It is your responsibility to request and submit this documentation.
Payment is required in full at the time of service. The payment on file will be charged unless otherwise agreed upon between provider and patient.
