Initial Evaluation: $375
80 minute visit. During this appointment, you will be asked about your current functioning or concerns, and history of physical or mental health issues. Your provider will use this information to establish a working diagnosis and treatment plan, which may or may not include medications and/or lab work.
Medication Management: $150
20 minute visit focusing on your current functional status and how prescribed treatments are affecting you.
Extended Medication Management Visit: $250
40 minute visit wherein you and your provider explore both pharmacologic & non-pharmacologic interventions to treat your condition. We may recommend more specialized counseling services if we feel that another treatment modality may better suit your needs.
Missed Visits: $75
To ensure every patient has access to timely care, we ask that patients only schedule appointments they intend to keep. Appointments canceled with less than 24 hours notice incur a $75 fee. This fee is the responsibility of the patient. Three or more missed or canceled appointments may be cause for discharge from the practice.
